Using the reporting plugin the following 

{color:#CCCCCC}{report-info:content:modifier > user:full name} {report-info:content:modification date|format=dd MMM, yyyy}{color}

outputs this:

{report-info:content:modifier > user:full name} {report-info:content:modification date|format=dd MMM, yyyy}
